Importing contacts act essentials

Hi

I am trying to import a csv into act essentials, but all it does is import the first two contacts and last contact of 23.  i have used the exact fields you have suggested by cutting and pasting so i know they are right.

I have been spending too long trying to get this to work now and it is driving me a bit crazy.  

The contacts it does import have no pattern as to why only those 3.

Please someone help!
